LAKELAND CARE
Functional Screener                                                                                       

The Functional Screener, as part of the Quality and Member Supports Division, serves enrolled members who meet the following target populations: frail elders, adults with physical disabilities and adults with intellectual/developmental disabilities. 

The position’s goal, under the general direction of the Long-Term Care Functional Screen (LTCFS) Supervisor, is to timely and accurately determine member eligibility for the Family Care Program per organizational policy and procedure while adhering to Department of Health Services (DHS) contract requirements.  The Functional Screener is responsible to conduct accurate, consistent, and detailed functional assessments.  These assessments serve as the foundation for the Comprehensive Strength Based Assessment completed by care management staff, provide a basis for care planning activities based upon Lakeland Care’s policies and procedures, and directly influence the organization’s capitation. 

In this position, Functional Screeners meet face to face with members to complete the functional screen and supplementary evidenced based screening tools, as needed.

Knowledge, Skills and Abilities:

  • A Bachelor’s degree is required, preferably in a health or human services related field.
  • A minimum of one (1) year of experience working with at least one of the pertinent target populations is required and knowledge of the long-term care programs.
  • Long-Term Care Functional Screen certification status must remain “in good standing”.
  • Excellent active listening and interviewing skills.
  • Strong interpersonal and communication skills, both verbal and written.
  • Ability to establish and maintain effective working relationships with stakeholders.
  • Ability to comprehend and apply critical and complex information.
  • Ability to be flexible and adaptable.
  • Working knowledge of computers, computer programs, typing, and data entry.
  • Ability to travel within the geographical service regions and throughout the state of Wisconsin. 
  • Ability to organize and plan work effectively and independently.
  • Ability to navigate challenging situations and take appropriate action.
  • Understanding of basic Care Management principles.

Current WI driver’s license, acceptable driving record and proof of adequate insurance required.

LAKELAND CARE
IDT Care Manager

The Care Manager, as part of an interdisciplinary team (IDT), serves frail elders, adults with physical disabilities and adults with intellectual/developmental disabilities who are members of Lakeland Care (LC).  The team’s goal is to promote the overall well-being of the members LC serves by providing high quality, person-centered, outcome based care.

Under general direction of the Care Management Supervisor, this position provides care management and service coordination to LC members.  As a member of the IDT, the care manager arranges for provision of services and supports based on a comprehensive assessment of the member's identified outcomes and needs. The IDT monitors the provision of services based on the member-centered plan per LC policy and procedures, and Department of Health Services (DHS) contract requirements.

Knowledge, Skills and Abilities:

  • Certified Social Worker in the State of Wisconsin, or
  • Bachelor’s degree in human services, psychology or related field with two (2) years’ experience working with elders or persons with disabilities preferred;
  • Working knowledge of computers, computer programs, typing, and data entry.
  • Ability to access members’ homes which are not required to comply with the ADA regulations.
  • Ability to multi-task and work in a fast paced environment.
  • Ability to lift up to 25lbs.
  • Current WI driver’s license, acceptable driving record and proof of adequate insurance.

In evaluating candidates for this position, Lakeland Care may consider a combination of education, training and experience which provides the necessary knowledge, skills and abilities to perform the duties of the position.  The education and or the experience requirement for this position may only be waived with the approval of the State as per LC’s contract.

Essential Job Functions

This position reports to the Area Director of Continuing Professional Education and will identify new workforce and professional development programs and other innovative educational needs and opportunities for business and industry and other targeted constituencies. This position will focus on priorities, objectives, marketing and outreach vehicles as they relate to all four campus locations to ensure adequate capacity for program support and instruction. In addition, this position will participate in regional and statewide networks to collaborate with business and industry representatives, professional colleagues, educational agencies, campus academic and administrative units, and other partners on workforce issues, professional development, educational needs, and community economic development.

Qualifications

MINIMUM QUALIFICATIONS:
  • Bachelor's degree in business, marketing, project management, adult education, or related field, at time of application
  • 3 years of experience in the development and management of self-supported, revenue-generating programs
  • Experience with developing and sustaining business relationships
  • Strong fiscal management, organizational, and computer skills

PREFERRED QUALIFICATIONS:

  • Master's degree in business, marketing, project management, adult education or related field
  • 5 years of experience in the development and management of self-supported, revenue-generating programs
  • Experience in organizational development and/or project management
  • Knowledge of current issues in adult and continuing educatio
